Tattoo, Body Piercing and Permanent Cosmetics Program

The Department of Public Health, Environmental Health Section registers all persons (practitioners) applying tattoos, body piercing(s), or permanent cosmetics. Department services include, but are not necessarily limited to, the following:

  • Issuing Permits
  • Conducting yearly inspections of all facilities for general maintenance and cleanliness, proper sterilization procedures
  • Maintaining registration documents of and issuing registration cards to all active practitioners
  • Promulgating (publicizing)  new or revising existing ordinances and regulations

Permit for business owner/operator:

A business owner/operator is someone who engages in the practice of tattooing, body piercing or applying permanent cosmetics. To apply for a business owner/operator permit:

  1. Fill out and bring the following forms:
    1. Application for Permit To Operate or Certificate of Sanitation (pdf)
    2. Zoning Referral Form (pdf)
    3. Fire Marshal Form (pdf)
  2. Include a check for $356 payable to the DPH (Department of Public Health)  for application fee.
  3. A check for $25.00 payable to the Tax Collector for the annual registration fee for practitioner permit.
  4. A completed Application for Registration Card (pdf)
  5. Bring above items and:
    1. A driver license or photo id
    2. A blood-borne pathogen training certificate
    3. A hepatitis vaccinations certificate

Permit for practitioners:

Practitioners are persons who are solely engaged in the practice of tattooing, body piercing or applying permanent cosmetics. To apply for a practitioner permit:

  1. A completed Application for Registration Card (pdf)
  2. A check for $25.00, payable to the Tax Collector.
  3. Bring
    1. A driver license or photo id
    2. A blood-borne pathogen training certificate
    3. A hepatitis vaccinations certificate

Temporary permits

Temporary permits are required for tattooing, body piercing or permanent cosmetics booth at special events. For a temporary permit, fill out and submit the following forms:

  1. Special events sponsor application for body arts booth (pdf)
  2. Vendor application to operate body arts booth (pdf)
  3. A copy of current registration or a completed Application for Registration Card (pdf)
  4. A check for $100 for a sponsor fee and corresponding fee for each high hazard booth. See worksheet to calculate the fees for the high hazard booth for special events (pdf)
