Tattoo, Body Piercing and Permanent Cosmetics Program
Permit for business owner/operator:
Business owner/operator is someone who engages in the practice of tattooing, body piercing or applying permanent cosmetics)
- Fill out and bring the following forms:
- Application for Permit To Operate or Certificate of Sanitation (pdf)
- Zoning Referral Form (pdf)
- Fire Marshal Form (pdf)
- Include a check for $356 payable to the DPH (Department of Public Health) for application fee.
- A check for $25.00 payable to the Tax Collector for the annual registration fee for practitioner permit.
- A completed Application for Registration Card (pdf)
- Bring above items and:
- driver license or photo id.
- blood-borne pathogen training certificate
- hepatitis vaccinations certificat
Permit for practitioners:
Persons who are solely engaged in the practice of tattooing, body piercing or applying permanent cosmetics
- A completed Application for Registration Card (pdf)
- A check for $25.00, payable to the Tax Collector.
- Bring
- driver license or photo id.
- blood-borne pathogen training certificate
- hepatitis vaccinations certificate
Temporary permits:
Tattooing, body piercing or permanent cosmetics booth at special events. Fill out and submit the following forms:
- special events sponsor application for body arts booth (pdf)
- vendor application to operate body arts booth (pdf)
- copy of current registration or a completed Application for Registration Card (pdf)
- a check for $100 for a sponsor fee and corresponding fee for each high hazard booth. See worksheet to calculate the fees for the high hazard booth for special events (pdf)
