Environmental Health

To apply for a permit to operate a:

• Dog Kennel
• Laundromat
• Pet shop
• Pet hospital

Step 1:

Print and Fill out the following 3 forms:

1. application for permit to operate or certificate of sanitation (pdf)
2. zoning referral (pdf)

   applicants for Laundromats do not need to fill form #2

3. fire marshal application (pdf)

Step 2:

Send all of the above forms and include a check for:

•  $359.00  for Dog Kennel, Pet Shop or Pet Hospital or
•  $254 for Laundromat

Fill out  Referral Inspection Request for a Fire Department Inspection

SEND THIS FORM to the San Francisco Fire Department. The address is included on the form.
