Home-Based Environmental Evaluation and Education for San Francisco Clients with Asthma


San Francisco Department of Public Health’s Environmental Health Section (EHS) welcomes medical providers to refer their patients with asthma for home-based environmental evaluation and education. This service consists of one home visit where the patients’ home environment is assessed for asthma triggers. At the end of the home visit, patients will be given educational materials, and recommendations and tools (such as free allergen-proof mattress covers, referrals to home help aid services, etc.) for controlling in-home asthma triggers. To receive this service, simply fill out the MEDICAL PROVIDER REFERRAL FORM and fax (415-252-3930) it back with the ASTHMA HOME ACTION PLAN to Helen Zverina or Olga Jaurigui.

 

Individual patients who want to receive this home-based environmental evaluation and education can discuss their interest with their medical providers.
