Asthma Home Visiting Program

Environmental Health staff can visit the home of asthma patients who are referred by their healthcare provider and conduct a one-time home survey visit, at no charge, to identify common environmental asthma triggers and help residents achieve solutions. At the end of the home visit, patients will be given educational materials, recommendations and tools (such as free allergen-proof mattress covers, referrals to home help aid services, etc.) for controlling in-home asthma triggers.

 

Requesting a Home Visit

If you would like to have an environmental health professional assess your home for asthma triggers, fill out the Medical Provider Referral Form (pdf) and fax it back with the Asthma Home Action Plan (pdf) to the attention of Haroon Ahmad. The fax number is (415) 252-3889.

Individual patients who want to receive this home-based environmental evaluation and education can discuss their interest with their medical providers.
