Request For Proposals (RFP) 31-2017
Employee Satisfaction Survey at Laguna Honda Hospital

Laguna Honda Hospital and Rehabilitation Center (Laguna Honda), owned and operated by the San Francisco Department of Public Health (DPH), seeks proposals for a contractor to conduct an Employee Satisfaction Survey.  This project will involve the contractor developing, formatting and implementing a survey for approximately 1700 healthcare facility employees measuring factors that contribute to overall job satisfaction.  The contractor's primary deliverables will be a comprehensive report of survey responses and data analysis.


This survey is intended to provide the organization with valuable information and constructive feedback concerning its work environment and work place practices.


The selected contractor will partner with Laguna Honda to develop and prepare the survey, store and transcribe the responses, and provide summary, analysis and reports.
