Get help to prevent falls and injuries in your home

The Community and Home Injury Prevention Program for Seniors (CHIPPS) is a fall prevention and education program.

What to do

Submit a Referral Form to CHIPPS@sfdph.org.

If you can't complete the online form then call us:  628-206-7695.

We will schedule an in-home safety assessment with you.

Services

Our injury-prevention specialist will come to your home and determine your safety needs.  If you need safety items, we can provide:

  • night lights
  • bathmats
  • kitchen timers
  • grabber tools

If you meet income eligibility we can arrange free home safety modifications.  Please see referral form for income eligibility.

Home modifications may include installation of:

  • grab bars
  • handrails
  • shower chairs
  • toilet rails

For those not income-eligible for home safety modifications we provide low-cost, reputable resources.
