Healthy Communities Together (HCT)


Healthy Communities Together (HCT) is a countywide collaborative of collective impact initiatives that are led by San Francisco community partners. The goals of HCT are to:

  • 1.) Develop a common agenda of health among major partners and communities
  • 2.) Promote institutional collaboration in community engagement
  • 3.) Empower collective impact collaboratives by policies, systems and environment change.

Community and city leaders recognize that health is influenced by multiple aspects of our community throughout our lives. HCT incorporates efforts that seek to address the social determinants of health from a life course perspective.
