CONFIDENTIALITY

Your records at City Clinic are always kept confidential. This means that information regarding testing, diagnosis and treatment services provided at the Clinic is privileged and will only be released to your treating physician upon request. You may request your records be shared with others by submitting a City Clinic Release of Medical Records form. Forms from other medical facilities will not be accepted.

City Clinic medical records are protected from release by Title 17, California Code of Regulations, Section 2636, which states:

Reports of examinations, cases, investigations and all records thereof made under the regulations for the control of venereal diseases shall be confidential and not open to public inspection and no part thereof divulged, except as may be necessary for the preservation of the public health.

All City Clinic staff sign an oath to keep confidential any and all information having to do with our patients. The oath specifically states that staff will not remove files from the Clinic without authorization and will not divulge any information regarding patients to unauthorized persons. Our staff understands that the unauthorized release of information will result in disciplinary action or termination of their employment with the City.
