The Budget Committee of the Commission will consider the consent calendar on this agenda at 1:00 p.m. at 101 Grove. The Budget Committee Consent Calendar consists of two sections. The first section is for items that will be considered for approval. There will be no separate discussion of these items unless a member of the Budget Committee so requests. The second section consists of items that will be for discussion and approval.

Consent Calendar items that have been recommended for approval by the Budget Committee will be acted upon by a single roll call vote of the Health Commission. There will be no separate discussion of these items unless a member of the Health Commission so requests, in which event the matter shall be removed from the Consent Agenda and considered as a separate item.

3.1 BHS – Request for approval of a retroactive contract renewal with Westside Community Mental Health Center, Inc, in the amount of $9,311,501, which includes a 12% contingency, to provide substance abuse treatment for the period of July 1, 2006 through June 30, 2010. (4 years)

3.2 BHS – Request for approval of a retroactive contract renewal with YMCA Urban Services, in the amount of $3,288,550, which includes a 12% contingency, to provide outpatient and prevention substance abuse and Proposition 63 mental health services for the period of July 1, 2006 through December 31, 2010, (4.5 years).

3.3 BHS – Request for approval of retroactive contract renewal with the Regents of the University of California in the amount of $3,147,347 to provide crisis intervention and stabilization, clinical case management, and linkage services for the period of July 1, 2006 through June 30, 2010. 3.4 PHP-Housing & Urban Health – Request for approval of a retroactive renewal contract with Ark of Refuge, Inc. in the amount of $1,882,675, which includes a 12% contingency, to provide transitional housing for Lesbian, Gay, Bi-sexual, Transgender, Queer and Questioning Young Adults between the ages of eighteen to twenty-four for the period of July 1, 2006 through June 30, 2010. 3.5 PHP Primary Care – Request for approval of a retroactive contract renewal with the Curry Senior Center in the amount of $1,174,965, which includes a 12% contingency, to provide comprehensive primary health care services for the period of July 1, 2006 through December 31, 2010 (4.5 years).

3.6 BHS – Request for approval of a new retroactive contract with Central City Hospitality House in the amount of $1,066,800, which includes a 12% contingency, to provide a drop-in center for mental health socialization and wellness services, for the period of July 1, 2006 through December 31, 2007. 3.8 CHN-Health Information Services – Request for approval of a new contract with Webmedx, Inc., in the amount of $550,000, to provide medical dictation and as-needed transcription services for San Francisco General Hospital Medical Center (SFGHMC), for the period of January 1, 2007 through December 31, 2007 (1 year).

3.11 BHS – Request for site approval for relocation of Citywide Case Management and Community Focus (UCSF/SFGH Department of Psychiatry Outpatient Programs) from current site at 939 Market Street, Fourth Floor, San Francisco, to 982 Mission Street, Second through Fourth Floors, San Francisco.

3.12 AIDS OFFICE-HIV Prevention – – Request for approval of a retroactive renewal contract with UCSF AIDS Health Project, in the amount of $700,000, which includes a 12% contingency, to provide HIV IDU case coordination services for individuals diagnosed with HIV or AIDS, for the period of July 1, 2006 through December 31, 2008. 3.13 PHP-Housing & Urban Health – Request for approval of a retroactive renewal contract with Self Help for the Elderly, in the amount of $557,352 which includes a 12% contingency to provide a bed day of care to 15 cognitively impaired seniors who have Alzheimer’s, Parkinson’s or multi-infarct dementia for the period of July 1, 2006 to June 30, 2010 (4 years).

3.14 PHP Primary Care – Request for approval of a retroactive sole source contract renewal with Lyon-Martin Women's Health Services in the amount of $379,184, which includes a 12% contingency, to provide comprehensive primary medical care services for the period of July 1, 2006 through December 31, 2010. 3.15 PHP-Housing & Urban Health – Request for approval of a retroactive renewal contract with Larkin Street Youth Services, in the amount of $287,889, which includes a 12% contingency, to provide Integrated Case Management services targeting multiply-diagnosed homeless young adults 18 to 25 for the period of July 1, 2006 through June 30 2010. 3.17 AIDS OFFICE-HIV Prevention – Request for approval of a retroactive renewal contract with UCSF AIDS Health Project, in the amount of $156,397, which includes a 12% contingency, to provide Intensive Case Management for Methamphetamine services for individuals diagnosed with HIV or AIDS, for the period of July 1, 2006 through December 31, 2008. 3.18 PHP – Environmental Health Section, Agriculture Program - Approval to retroactively accept and expend two grants from the California Department of Food and Agriculture in the amounts of $63,888 and $42,076 (for a total of $105,964) in support of the Pierce’s Disease Control Program and the High Risk (Pest Exclusion) Program for the period of July 1, 2006 to June 1, 2007.
