The Budget Committee of the Commission will consider the consent calendar on this agenda at 2:00 p.m. at 101 Grove. The Budget Committee Consent Calendar consists of two sections. The first section is for items that will be considered for approval. There will be no separate discussion of these items unless a member of the Budget Committee so requests. The second section consists of items that will be for discussion and approval.

Consent Calendar items that have been recommended for approval by the Budget Committee will be acted upon by a single roll call vote of the Health Commission. There will be no separate discussion of these items unless a member of the Health Commission so requests, in which event the matter shall be removed from the Consent Agenda and considered as a separate item.

3.1 BHS – Request for approval of a retroactive contract renewal with Polaris Research & Development, in the amount of $2,060,302, which includes a 12% contingency, to provide Black Infant Health Improvement Program services targeting African American parents for the period of July 1, 2006 through December 31, 2010 (4.5 years).

3.2 ADMINISTRATION - Resolution Authorizing the Department of Public Health, Laguna Honda Hospital & Rehabilitation Center (LHH) to accept a gift of an electric cart valued in the amount of $11,091.68, from the Laguna Honda Hospital Volunteers Inc. to be used for the maintenance and operation of the animal assisted therapy program and related facilities.

3.3 BHS – Request for approval of a retroactive contract renewal with the Regents of the University of California Center on Deafness, in the amount of $509,593, to provide mental health and substance abuse outpatient services for the period of July 1, 2006 through June 30, 2007 (1 year).

3.4 CHN – Rehabilitation - Request for approval of four (4) contract renewals with the following two (2) firms: On-Call Therapists, Inc. and Preferred Healthcare Registry, Inc. in the combined amount of $702,750 to provide intermittent, as-needed rehabilitation therapy personnel services (Physical, Occupational and Speech Therapists) for Laguna Honda Hospital for the period of February 1, 2007 through January 31, 2009 (2 years) and for San Francisco General Hospital for the period of February 1, 2007 through January 31, 2011 (4 years).

3.5 AIDS OFFICE-HIV Prevention – Request for approval of a retroactive renewal contract with Legal Services for Children, in the amount of $465,202, which includes a 12% contingency, to provide legal/guardianship/children services for individuals diagnosed with HIV or AIDS, for the period of July 1, 2006 through December 31, 2010 (4.5 years).

3.6 PHP-Housing & Urban Health – Request for approval of a contract modification with Lutheran Social Services in the amount of $100,258 for a combined total of $567,945 which includes a 12% contingency to provide money management and third party rent payment services to residents of the Direct Access to Housing (DAH) sites for the period of July 1, 2006 through June 30, 2008. (2 years).

3.7 PHP – Primary Care - Request for approval to enter into an agreement in the amount of $241,011 with Tenderloin Health, the primary contractor, to participate in a project to expand oral health services for homeless individuals infected with HIV disease in San Francisco’s Tenderloin district, for the period of April 1, 2007 through December 31, 2007.
