The Budget Committee of the Commission will consider the consent calendar on this agenda at 1:30 p.m. at 101 Grove. The Budget Committee Consent Calendar consists of two sections. The first section is for items that will be considered for approval. There will be no separate discussion of these items unless a member of the Budget Committee so requests. The second section consists of items that will be for discussion and approval.

Consent Calendar items that have been recommended for approval by the Budget Committee will be acted upon by a single roll call vote of the Health Commission. There will be no separate discussion of these items unless a member of the Health Commission so requests, in which event the matter shall be removed from the Consent Agenda and considered as a separate item.

3.2 DPH Central Administration – Request for approval of a resolution extending the terms of Fiscal Year 2006/07 contracts through the first six months of Fiscal Year 2007/08.

3.4 PHP-Community Behavioral Health Services – Request for approval of a resolution authorizing the Department of Public Health, Community Behavioral Health Services to enter into a four-year Combined Net Negotiated Amount and Drug Medi-Cal Agreement with the California State Department of Alcohol and Drug Programs, in the amount of $55,825,476, for the period of
July 1, 2007 through June 30, 2010.

3.5 PHP-BEHM – Request for approval of a contract modification with General Environmental Services, in the amount of $194,400, for a new amount of $414,400, which includes a 12% contingency, to provide hazardous waste management and transportation services, for the period of August 1, 2004 through June 30, 2008 (3.92 years).

3.6 PHP-BEHM – Request for approval of a contract modification with Veolia Environmental Services, LLC, in the amount of $210,840, for a new amount of $568,340, which includes a 12% contingency, to provide hazardous waste management and transportation services, for the period of August 1, 2004 through June 30, 2008 (4 years).

3.7 PHP-BEHM – Request for approval of a contract modification with Raibon and Colbert Associates, Inc., in the amount of $180,000, for a new amount of $280,000, which includes a 12% contingency, to provide hazardous waste management and transportation services, for the period of April 1, 2005 through June 30, 2007 (3.25 years).

3.8 CHPP – Request for approval of a contract renewal with AmeriChoice, in the amount of $428,062, which includes a 12% contingency, to provide fiscal intermediary services to Personal Assisted Employment (PAES) dental services, for the period of July 1, 2007 through June 30, 2008 (1 year).

3.9 CHN-SFGHMC Nursing Operations – Request for approval of seven (7) contract modifications with the following firms: Agostini Medical Staffing, Arcadia Health Services, Inc. dba Arcadia Staff Resources, Crdentia Corporation, HRN Services, Inc., Medstaff, Inc., Nurse Providers, Inc. and Professional Resource Enterprises, Inc. dba United Nursing International, to increase the shared contract amount by $750,000, from $2,700,000 to $3,450,000, to provide supplemental, temporary Per Diem and Traveling nursing personnel services for San Francisco General Hospital Medical Center, for the period of July 1, 2006 through June 30, 2007 (12 months).

3.10 BHS – Request for approval of a retroactive contract modification with Tenderloin Health, in the amount of $73,436, to add substance abuse services targeting methamphetamine users in San Francisco. Total amount of contract is $274,794, including 12% contingency for the period of July 1, 2006 through December 31, 2007 (18 months).

3.11 CHN–San Francisco General Hospital, Health at Home – Request for approval to accept and expend retroactively a new grant from the Institute on Aging in the amount of $578,842 for the San Francisco Department of Public Health, Health at Home to provide comprehensive nursing case management, physical therapy and occupational therapy home safety evaluations, and nursing consultations, for the period of April 1, 2007 through June 30, 2009.
