Resolution:

Approving a Policy On Contract Modification, October 7, 1997

WHEREAS, the Department of Health's operating divisions currently follow different procedures to modify contracts; and,

WHEREAS, the Department is working to standardize its contract policies and streamline contracting procedures; and,

WHEREAS, the Department has proposed a contract modification policy requiring that substantive changes to contracts, including but not limited to changes to total cost, legal entity, term, reimbursement methodology or program scope would require approval from the Health Commission, City Attorney, Purchaser, Director of Administrative Services, and Controller; and,

WHEREAS, the Department has also proposed that minor changes to contracts, including but not limited to changes in budgets, line items, and program units of service may be implemented administratively by Department managers, with written justification and final approval from the Department's Deputy Directors; and,

WHEREAS, the Health Commission supports the proposal because it standardizes the contract modification process department-wide, establishes and assures accountability, and streamlines the implementation of minor contract revisions, and,

WHEREAS, the proposal has been approved by the City Attorney and is consistent with the City's laws and regulations regarding contracting; therefore, be it

RESOLVED that the Health Commission approves the Department's policy on contract modifications.
