Resolution:

Resolution Reaffirming the Department of Public Health's Vision Statement and Restating the Mission Statement, September 1, 1998

WHEREAS, the Department of Public Health has both a Vision Statement and Mission Statement; and

WHEREAS, the Vision Statement provides the guiding principles and values of the Department of Public Health and the Mission Statement defines the Department's fundamental responsibilities in carrying out the vision; and

WHEREAS, the Department of Public Health has responsibility for overseeing the health of the entire community and a role in providing comprehensive health care services; and

WHEREAS, the Health Commission adopted a Vision Statement for the Department of Public Health via Resolution Number 4-93 and believes that the Vision Statement is still appropriate;

WHEREAS, the following Vision Statement for the Department of Public Health is being affirmed:

San Francisco will be a leader in health.  The staff and volunteers for the Department of Public Health will do everything in their power to help all San Franciscans achieve the best possible state of health.  We are committed to making this a city where:

WHEREAS, the Mission Statement is being updated to ensure that it accurately communicates and describes the Department of Public Health's fundamental role and responsibilities; and

WHEREAS, the following Mission Statement has been proposed by the Department of Public Health:

The mission of the Department of Public Health is to protect and promote the health of all San Franciscans.

The Department of Public Health shall:

RESOLVED, that the San Francisco Health Commission reaffirms the Department of Public Health's Vision Statement as adopted by Resolution Number 4-93; and be it 

FURTHER RESOLVED, that the San Francisco Health Commission adopts the Department of Public Health's restated Mission Statement; and be it

FURTHER RESOLVED; that this Mission Statement replace all previous Mission Statements.

I hereby certify that the foregoing resolution was adopted by the Health Commission at its meeting of Tuesday, September 1, 1998.
