Resolution:

Approving the Formation of the Medical Practice Group of the Community Health Network, Resolution 38-98, December 15, 1998

WHEREAS, the City and County of San Francisco, Department of Public Health, has reorganized to create an integrated health care delivery system; and,

WHEREAS, the Department of Public Health delivery system, named the Community Health Network of San Francisco (CHN), includes primary care, emergency services, specialty services, acute care, home care, long term care, mental health services, substance abuse services, and forensics; and,

WHEREAS, the providers within the Community Health Network desire to organize themselves into a practice group with a collective and independent voice for making recommendations to the CHN Leadership Group, Director of Health, and Health Commission, regarding matters such as policy, budget, high level personnel decisions, space allocation, and managed care contracting; and,

WHEREAS, the providers propose that this practice group be known as the Medical Practice Group of the Community Health Network; and,

WHEREAS, the Health Commission has previously supported the direction of developing a practice group for the CHN; now, therefore, be it

RESOLVED, that the Health Commission hereby approved the formation of the Medical Practice Group of the Community Health Network, and approves the bylaws of the Medical Practice Group.
