Thank you for visiting this page. This web page allows City Clinic patients who were seen in clinic before Tuesday, March 8, 2022, to access their test results.

If you were seen in clinic any time AFTER 3/7/22 and want to view your results, please sign up for MyChart. MyChart will allow you to see your test results and visit notes at SF City Clinic and any other SFDPH clinic. Follow the directions at this link or review this document.

You can also call 628-217-6695 and leave a message to get your results. A City Clinic staff person will return your phone call within 5-7 business days.

To protect confidentiality, no information identifying the patient will be displayed other than the referral number entered below.
