ZSFG-Cancer Navigation Program

Monday - Friday: 8:30am - 5:00pm

 

Mission:

Dedicated to the support and care of the diverse and underserved populations who have been diagnosed with cancer at ZSFG. Our team of clinicians and navigators provide a wide range of services that promotes wellness for patients as they navigate through diagnosis, treatment and survivorship.

Services are free and available in Spanish, Cantonese and Mandarin

Our navigators are part of your comprehensive care team and provide:

  • Provide emotional support and foster a sense of community among people affected by cancer
  • Community & hospital-based navigation/ case management throughout the continuum of care
  • Provide emotional and psychosocial support that promote wellness and speak to the diverse needs of our patients
  • Coordination of care with medical providers, community agencies, and support staff
  • Accompaniment to appointments
  • Health education including: diagnosis, treatment & survivorship
  • Patient advocacy
  • Referrals to community- wide, culturally appropriate services including:

  • Emergency financial assistance
    Food assistance
    Transportation
    Wellness groups
    Support groups

Services are free and available in Spanish, Cantonese and Mandarin

  • Communication with patient's health care team to promote cultural understanding
  • Schedule appointments for treatment and follow-up care
  • Coordinate interpreting services
  • Provide education on overall healthcare recommendations
  • Provide emotional support
  • Refer patients to culturally appropriate support groups
  • Assist with practical problems related to treatment (such as transportation, medical bills, childcare, financial assistance, wigs and prosthetics, home healthcare, pain management and exercise)
  • Listening, caring and supporting patients in their time of need
